Manual payment

To offer any upgrades i need to set a payment provider.
I dont want to set up a payment provider.
I want to show this page like:

One month 10 dollar.
One year 100 dollar.

People should choose (and maybe get a code)
Then they send my money with x, y or z and send a username (or code)
I watch my bank account and see code or username, go to my acp and manually finish the order.

I am sure there are many people who need something like this.

Now I have to set up upgrades, but don't show them...
