It used to be from PlayDotA.com which was a site from the early 2000s, I've been told it was a XenForo, uh, forum, so I'm guessing this smilie came from an ancient smilie set here, possibly even a default set.
Might be a stupid question. Might've just been custom-made. It's just funny 'coz this smilie's been in my friend group for a decade now and since PlayDotA.com died, we're the last 7 people who still use it extensively or even at all.
