approving moderation queue twice?

I have to approved items in moderation queue twice. I check them off, I approve them, and then it looks like it goes thru - but it doesn't. The items stay in approval queue and I have to do it a second time.

Is there a setting I can adjust, or something I can fix, so that moderated items only need admin approval once?
